It is necessary for the school to keep information on hand so that parents or other responsible adults can be contacted in case of a medical emergency. Also, if parents are out of town or if they cannot be located, the school needs some specific directions in order to deal with the emergency. To help us do this, each student needs to have an Emergency Medical and Dental Form on file at school. You filled out an Emergency Medical and Dental Form at resgistration. Please keep this updated by informing the secretary and teacher of any changes.
