Cafeteria Plan

Expenses for qualifying categories are paid with pre-tax dollars. Option to pay for insurance premiums (Medical, Vision, Dental, Long-Term Disability. Options to pay expenses for Child Care (The employee must submit form for reimbursement.) Options to pay expenses for Unreimbursed Medical expenses (including co-pay, deductibles, medications and over-the-counter health/medical purchases). The employee must submit form for reimbursement. Employee makes changes in monthly deduction amounts annually at end of calendar year.

NOTE:  Deducted amounts that are not used will not be refunded, per IRS regulations.
