Please keep emergency contacts up-to-date! You filled out a blue form called the "Emergency Medical Authorization Form." This form lists alternate emergency contacts, doctor, dentist, and hospital. It is vital that the school have current contacts in the event of an emergency involving your student's health.
This form also has "Check Out Permission" information. ONLY THE PEOPLE LISTED ON THE SHEET MAY SIGN OUT THE STUDENT. Please do not call the office or write a note authorizing anyone else to check out your student. The office staff is not allowed to let the student leave with anyone who is not listed on the blue emergency information sheet. If you need to add names to the sheet, please come into the office.
