WHAT TO DO IF YOU ARE INJURED AT WORK

  1. Contact your Supervisor/Principal/Administrative Assistant immediately.

  1. If your Supervisor/Principal/ Administrative Assistant is not available, call the Finance Department at Central Office. 769-4327. Forms can be submitted to laurel.jaime@clovis-schools.org

  1. YOUR WORK RELATED INJURY MUST BE REPORTED WITHIN 24 HOURS.

  1. Be sure to complete the required forms—even if you do not need medical attention.  Your Supervisor/Principal/Administrative Assistant must also complete forms relating to your injury.

  1. If your injury requires Emergency treatment, notify a Supervisor and then you should go to the Emergency Room.

  1. If an injury requires medical treatment, after notifying your Supervisor/Principal/Administrative Assistant, call for an appointment. You may select any Medical facility that accepts Worker’s Compensation. Employees cannot go out of state for medical treatment for any work related injury.

  1. If you need to see a Doctor for a work injury and you do not have a regular Doctor, you should go to the Urgent Care Center at Plains Regional Medical Center.  After treatment, they may refer you to a local Doctor or Specialist for follow-up treatment.

  1. NEVER GIVE YOUR MEDICAL I.D. CARD TO A DOCTOR FACILITY, HOSPITAL, OR PRESCRIPTION SERVICES FOR A WORK RELATED INJURY.

  1. If you receive medical treatment for an injury, you must take a Doctor’s note to the Employee Service Department at Central Office.  You will not be allowed to return to work without a release from the doctor.
